Form value complications.

I client wants this form on their insurance site.

Which is easy to do. It’s made to be put in an iframe for other peoples use on their site. We have the license and such.

Anyway, here is the issue. They want the zip code box on the form to be on the home page so you can type it in there just like this site:

http://calhealth.net/

You type in the zip code there and it brings you to the second page of the form. I have absolutely no idea how they do that. Anyone have any ideas?
