We’re not a tutorial site but your request is a little vague.

I think you are asking for a pop-up modal box.

From a screenshot, we can’t see what the script does. We only see a popup modal and your description “moving from this box filed to another or another” is still somewhat vague.

What you appear to be asking is beyond the usual advice we give here.

It’s quite likely that it’s a custom-built piece of Javascript and not just something you can plug in to a site and have it miraculously have work the way you want it to.

If you find one and can’t get it working right then we might be able to help.
