submit button on form won’t show up

Can’t figure this one out. The submit button won’t show up on my contact form. I just pasted the form code into a simple html doc to make things simple.

Yup that was it… I used a contact form generator http://www.addressmunger.com/contact_form_generator/

The Code was a total mess… I wonder if there is another contact form generator with captcha.
