Need simple PHP Form with Image Upload option…!

Hi Guys,

I’ve been hunting around for a simple form I can set up with the following fields…

• Name[/*:m]
• Email[/*:m]
• Image (Upload)[/*:m]
• Image Description[/*:m]
• Submit[/*:m][/list:u]

  I’ve found heaps of good php forms, and php image upload scripts, but none that combine the two.

  It’s probably pretty easy to combine the two(?) but I’m not that great with PHP…!

  If anyone would point me in the right direction, I’d be much appreciative!

I ended up using FormM@iler. It was a bit tricky configuring it for uploads, but it all worked out in the end!
