help with #62: Advanced Form Styling, Select boxes

Hi All

I have successfully implemented the tutorial on form styling and validation

I have tried to also use it on a different page but using a select box instead of a check box to show and hide additional input boxes. But this does not work when the form styling is added??? with out the form styling it works fine.

I can give you links if you want to have a look. any advice would be greatly appreciated.
