    I work for a non-profit involved in collection data on arthritis patients. Most of the data is collected old fashioned way i.e. patients filling out 25 page long questionnaire every 6 months and mailing it back to us.

    We have been providing online version of our paper questionnaire for patients to fill out. This has been working fine in terms of functionality etc but look and feel lacks major updating.

    I am looking for some feedback and help in getting our survey looks much better. I can not use Flash since it has to be supported on all browsers.

    Typical survey have lots of radio buttons, check marks , visual analog scales ( could be a slider using JavaScript?) ,,

    Anyone have a CSS template i can use for question, response and other tags used in a typical survey?
    I suppose i would need graphic and images files to go with that too

    Any input is highly appreciated

    i guess no one have any experience with CSS for Survey design ?

    Can anyone point me to the right direction where i can learn it on my own? Or may be pay someone to do a small project for me ?

    @hassanmir – Try Wufoo. I’m pretty sure they have survey functionality in their forms you can embed. They also have a HTML & CSS file you can download to design your own.

    Maybe some nice readings are the following articles Chris wrote. I used some of them myself when creating forms and I found them really helpfull, cause I think readability is really important in a form.

    Thanks, i will go over these links ,,

    Could you also point me to 1 or two books on the subject , i prefer a book that covers not only CSS but may be some supporting technology too? I looked at Amazon but there are too many books to find the right one(s)

    I have honestly never read a book about html/css/anything-that-has-to-do-with-webdesign before.. I’m not even creating websites professionally, just for fun and sometimes for friends or whatever. I’m sure other people can recommend good books though.
