Create Order Form for Catalog with Price and State Tax Calculations

I'm hoping to find an existing form I can modify or some type of form builder I can use to create a product(s) order form. I've found things like js.recurly.com and jquery-order-form-builder, but they seem like overkill for my needs.

Also. Payment function isn't a requirement.

Any suggestions?
